What Went Wrong With The Silver State Insurance Exchange?

The Silver State Health Exchange was supposed to deliver a big range of choices for people who needed to buy health insurance to comply with the Affordable Care Act. Now, it seems that the contractor, Xerox, was asleep at the switch and that's leaving thousands of Nevadans without health insurance. Or at least the exchange cannot tell them if the insurance company got their information and their money, so they don't know if they have insurance. So what went wrong? And is anyone really trying to fix it?
