The Nevada Occupational Safety and Health Administration issued six citations against Cirque in its investigation of the death of Sarah Guillot-Guyard during a June performance of Ka. The agency said the company failed to prevent performers from striking the grid, which dislodged Guillot-Guyard's cable, and didn't properly train employees.
