Caesar's Fined for Underage Gambling

The Nevada Gaming Commission fined Caesar's Entertainment Corporation $100,000 for underage gambling.  The commission said there was a "pattern" of abuse, and that another incident would mean a seven-figure fine.  So what happened?  Is Caesar's lax with its rules?  Or did teens cleverly slip past security?  And what can be done about underage gambling in the future?
